Almost always per arch. Upper and lower arch each priced separately. Full-mouth (both upper and lower) is typically 1.7-1.9x single arch cost - not exactly 2x because some planning/consultation overhead is shared. Confirm with your provider whether the quote is per arch or full mouth.
A single tooth implant runs $3,000-$5,000. Replacing 14 teeth individually would be $42,000-$70,000 per arch. All-on-4 uses just 4 implants to support a full bridge of teeth - far less surgery, less material, and often no bone grafting required because the tilted placement uses denser bone.
